In the fatal boat accident involving the tugboat Caribbean Sea and a tour boat, the
National Transportation Safety Board determined that the tug's mate failed to
maintain a proper lookout. The accident took place shortly after 2:30 pm on July 7,
2010 on the Delaware River. The tug was pushing an empty 250 foot long barge
that collided with the tour boat, which was anchored at the time because of
mechanical problems.
The NTSB states that its investigation revealed that the tug's mate was inattentive
to his duties while navigating the vessel. It mentions that he was distracted by
repeated use of a cell phone and lap top computer in communicating with his
family who were dealing with a family emergency. The tug had an upper
wheelhouse, which the investigation revealed was not used. The upper
wheelhouse would have provided better visibility.
Twenty six of the tour boat's passengers were injured and two were killed in the
tragic accident. Read the recent NTSB press release . There were no injuries
reported aboard the tug.The accident investigation also revealed that
maintenance personnel for the passenger vessel had not ensured that pressure
cap on the surge tank was securely in place before returning the vehicle to
passenger service. The NTSB indicated that this allowed the engine to overheat,
leading to the master stopping the vessel and anchoring in an active channel.
